GLP-1s Are Changing the Game—But Strength Matters Most

GLP-1 medications like Ozempic, Wegovy, and Mounjaro have become increasingly common in weight loss and diabetes care. For many people, they’ve been incredibly effective tools for lowering blood sugar, reducing appetite, and supporting meaningful weight loss.
Maintaining a healthy weight can improve long-term health. Weight loss has been linked to lower risks of heart disease, type 2 diabetes, and other chronic conditions.1
But healthcare professionals are also paying closer attention to another part of the conversation: preserving muscle and bone health during rapid weight loss.
When weight drops quickly, the body can lose muscle and bone density alongside fat. That’s why strength training and proper nutrition are so important in supporting overall health during a weight loss journey.

How GLP-1 Medications Work

GLP-1 medications mimic a hormone called glucagon-like peptide-1, which the body naturally releases after eating.2 This hormone helps regulate blood sugar and appetite in a few ways. For one, it signals the pancreas to release insulin, helping move sugar out of the bloodstream and into the body’s cells.
It also suppresses glucagon, a hormone that raises blood sugar levels when the body needs more energy. Together, these processes slow stomach emptying and help people feel fuller for longer after eating.
As a result, many people consume fewer calories throughout the day, which often leads to weight loss. While these medications were originally developed to help manage type 2 diabetes, their effects have made them popular for use in obesity treatment and weight management.
Many people taking GLP-1 medications experience notable weight reduction over time, especially when paired with lifestyle changes like improved nutrition and regular physical activity. But researchers are also continuing to study how rapid weight loss may affect muscle mass and bone density over time.

Weight Loss Isn’t Only Fat Loss

One reason this topic has gained more attention is that weight loss doesn’t come from fat alone. When people lose weight, they often assume the body is burning fat specifically. In reality, that weight loss can actually include reductions in muscle mass and bone density, especially when it happens quickly.

Muscle Loss with Rapid Weight Loss

Some muscle loss during weight loss is normal; it’s considered normal for up to ¼ of the weight lost to be fat-free mass.3 But when too much lean muscle is lost, people may start to notice changes in their strength and energy levels over time. Daily tasks like carrying groceries, getting up from the floor, climbing stairs, or maintaining balance become harder when muscle mass declines.
Muscle tissue supports the body during movement. Losing too much muscle can gradually make those movements feel harder and less stable.
Researchers are still studying exactly how GLP-1 medications affect body composition long-term. A recent review published in Circulation suggests that some of the weight loss while taking these medications may come from lean mass rather than fat alone.4 At the same time, experts note that some muscle changes may be a normal response to weight loss itself.
Even so, many health care providers encourage those taking GLP-1s to focus on strength training and adequate protein intake during weight loss.

Bone Density and Long-Term Health

Muscle isn’t the only tissue affected during weight loss. Researchers are also looking at how rapid weight reduction may affect bone density over time, especially in older adults and postmenopausal women who already face a higher risk of osteoporosis.
Bones constantly rebuild and strengthen themselves in response to stress and movement. Weight-bearing activity and strength training help stimulate that process.5 But when body weight drops fast, especially alongside reduced muscle mass or lower nutrient intake, bone density may decline as well.
A recent study published in The Journal of Clinical Endocrinology & Metabolism has found a modest increase in fracture risk among older adults taking GLP-1 medications for type 2 diabetes.6 While researchers are still studying the connection, the findings add to ongoing discussions around bone health during rapid weight loss, especially among groups already at higher risk for osteoporosis.

Why Strength Training Matters

One of the best ways to help protect the muscles and bones during weight loss is through resistance training. Exercises that involve resistance band work, weight machines, and free weights place stress on the muscles, signaling to the body that the tissue is still needed. That becomes especially important during periods of reduced calorie intake, when the body may break down muscle alongside fat.
Resistance training also benefits bone health. Bone responds to stress much like muscles do. When weight or force is applied to bone during exercise, the body responds by rebuilding and strengthening bone tissue over time. Research published by the National Library of Medicine has found that this type of mechanical load is important for maintaining bone density, particularly as people age.7

But not all exercise creates the same response. While activities like walking and cycling offer important cardiovascular benefits, resistance training places a greater load on both muscles and bones. That added resistance helps stimulate the body’s natural bone remodeling process while preserving strength and lean muscle mass.

Nutrition’s Role in Preserving Muscle and Bone

Food intake often changes substantially while taking GLP-1 medications. Many people feel full faster, eat smaller portions, and snack less throughout the day. That reduction in appetite can help with weight loss, but it can also make it harder to get enough of the nutrients the body needs to maintain muscle and bone tissue.
Protein becomes especially important when people are eating less overall. Without enough protein, the body has a harder time maintaining muscle mass during weight loss. Calcium and vitamin D matter for a similar reason. Bone tissue constantly rebuilds itself, and those nutrients support that process.8,9
Eating less doesn’t reduce the body’s need for nutrition. Eating enough protein, staying hydrated, and including nutrient-dense foods throughout the day can help support overall health while the body adjusts to lower calorie intake.

Who Should Pay Extra Attention?

edication can benefit from paying attention to muscle and bone health during weight loss. But for some groups, that conversation is especially important.
Older adults naturally lose muscle mass and bone density with age, even without significant weight reduction.10,11 Postmenopausal women also face a higher risk of osteoporosis because hormonal changes can accelerate bone loss over time.12 Rapid weight loss may intensify those changes.
People who are sedentary or have conditions like osteopenia, osteoporosis, or sarcopenia may also need to be more intentional about preserving strength and mobility while losing weight.

That doesn’t mean that GLP-1s aren’t an option for these groups; it just means muscle and bone health should remain part of the conversation throughout the process.

Sustainable Weight Loss Means Supporting the Whole Body

Weight loss can improve many aspects of health, and GLP-1 medications have become valuable tools for helping people reach that goal. But the rapid weight loss associated with these medications can place added stress on muscle and bone tissue if the body isn’t properly supported along the way.
Strength training and proper nutrition can help the body adapt more effectively to those changes. Over time, maintaining muscle strength and bone density may help people continue to move comfortably and remain physically independent as they age.
Ultimately, the number on the scale is only one piece of the puzzle. Feeling strong, moving easily, and maintaining long-term mobility all contribute to overall quality of life.
